The Dementia Reablement Team will support you following your initial diagnosis of early-stage Dementia from the Memory Clinic. We provide information and practical advice to enable you to continue to enjoy a good quality of life with your family and friends, to feel confident to remain independent and stay safe within your own home. We can help you to access your local groups and activities and provide information on dementia friendly shops and leisure activities. We can advise on assistive technology and using Smart phones or tablets to assist with everyday tasks. We can also provide support and advice for those people who support you. The service is available during weekdays and is flexible so support can also be provided during the early evening.
The office is open Mon – Thurs 9am – 5pm and Fri 9am – 4.30pm.

Accepted referral methods: Self referral. Via GP or Health Professional, Adult Social Care and Voluntary Agencies.
Eligibility: Cheshire East Resident with a diagnosis of early stage Dementia for those aged 18 years and over.
There is no cost to this service.
